If you are on the freeway and the car in front is going 60mph and they slam on their brakes and hit the car in front of them and you are going the same speed but you get down to 40 or 45 mph and can not move into the left or right lane to get over.And you have no choice but to hit them in the rear is that a chain reaction crash and if so who is at fault?

Everyone who couldn't stop in time would be at fault for not maintaining a safe distance between themselves and the vehicle in front, this kind of accident frequently happens in wet and foggy driving conditions, it's called a multiple shunt in the UK. |

Each car that is following to close to stop pays for the persons car in front and so on. See the two second rule still applies. Keep distance to react and stop that way you only have to worry about the girl doing her eye make up in the vanity mirror while texting her friend. |

If you don't tailgate or cut into the safety interval of other drivers, that won't happen...2 second rule applies here...and the twelve second rule also...when driving you should be looking 12 seconds down the road, so those pesky situations don't sneak up on you...yeah, it would be your fault for following too closely... |

Fault is not an absolute. The law and insurance companies decide on percentage of fault.
Someone who can't stop fast enough and has an accident when the driver in front of them stops very suddenly may only be 50% at fault, even if they were following too closely. Maybe 20%. Maybe 80%.
It depends a lot on conditions, time of day, traffic and driver experience.
Where one driver sees the problem and tries hard to stop and hits the guy, a more experienced driver leaves himself an 'out', and would take advantage of it if he couldn't stop in time and avoid an accident.
Was the guy REALLY following too closely? Did he hit the brakes? How long were the skid marks? Were there four of them? Was there room to change lanes? Was it foggy? How fast was he going? Was he tired? been drinking? Did the guy in front stop for a hazard visible to the second driver?
If the guy in front just stopped without any reason, he might be cited for reckless endangerment (or he might be dead), but that doesn't absolve the guy following too closely.
In a 'chain reaction' accident every driver might be more than 50% at fault.
