Excellent question. Back in the day when mechanics were much better (lol) the driver, and all the equipment the driver usually had in the car, for example, a travelling salesman, a contractor, as well as a full tank of gas was part of an alignment. Unfortunately, today many people are overweight and obese, so much so that it needs to be taken into consideration when an alignment is performed. If the garage won't allow it, go somewhere else.

A person large enough to throw the dynamics so far out of whack as to require special alignment would also require special suspension, and special tires. But that is all stupiridiculame because a person that large couldn't fit into any of the cars on the road today, he would be too large to walk period. He would be, at minimum, close to 800 lbs.

Let's look at it logically. Five 200 lb people ride in a car, that is 1,000 lbs, nobody worries about it.

Its not funny but yes we have to make changes to the alignment for larger people.

Well, that is a nice way of putting it. (sarcasm) I would have to say it depends on their size. There are some that do have their cars made custom because of their weight and height. Shaqille O'Neal is a prime example. He cannot ride in a car that is standard size for most people. If you are overweight then you shouldn't have to worry about the vehicle being aligned. But again it depends no how much overweight. Lets just say a person is 400 Lbs. They wouldn't necessarily have to have their vehicle changed unless their weight impacted the performance of the car or they were not able to fit in their vehicle. But to answer you question it all depends on how large the person is.
