has anyone ever heard of a fatal bicycle vs. telephone accident? is it even possible since the average person can only peddle up to 10 -20 mph at the most?

I have never heard of this particular situation, but it IS possible. Cyclists have run into walls, cars, other cyclists and other obstacles and there have been fatalities. This is especially true of those that don't wear helmets.
People have fallen while walking or running, hit their heads and suffered fatal trauma. |

I have not personally heard of a fatal collision with a telephone pole, but I also think your speed estimate is quite unrealistic. On flat terrain with a modern road bike, even an average cyclist can reach 20 mph fairly easily, and 40-50 mph on descents.
If you land wrong, particularly with no helmet, even a 0 mph tipover can be fatal. |

you're right but what if you're going downhill, at least the speed is going to double so you would be going at 40 mph |

Yes it is possible at any age or any speed if you hit a telephone pole with a bicycle. |
