What would cause a seatbelt to fail a vehicle inspection?

Since I have been inspecting vehicles for over 30 years the main reasons that seat-belt would fail the inspection would be.
Front lap seat belts are required and not present.
Seat belt webbing is frayed, split, or torn.
Belt anchorages or attachment fittings are loose, badly corroded, missing, or not fastened to belt.
Belt buckles loose or inoperative.
All seat belt anchor bolts are not securely fastened to floor or are missing.
Pelvic restraint is not present.
Seat belt will not adjust to allow proper fit. |

One other reason: if they have been altered or damaged in any way, like punching holes in the belt or using them to tie objects down. |

Because they are standard safety equipment, the seatbelts cannot be removed from the vehicle and must be in good working condition. This includes, no excessive wear like fraying, cuts, broken buckles, rusted out mounts, as well as retracter or adjustment problems. |

I can think of no reason why it should fail as long as you can put it on and buckle it. |
