or the people weren't badly hurt that they had to get to hospital in a hurry

if the person was DOA (dead on arrival) that person gets taken away in the coroner's vehicle. if the ambulance leaves with no lights or siren, it may have been a patient refusing treatment, or the person wasnt hurt very bad. the ambulance is going to the hospital "non-emergency"

if they have a patient aboard it means the patient is stable and they don't
need to rush to the hospital ( like a broken leg for example)

It either means there was nobody to take away, or that the person(s) taken away were deceased.

I was going to say listen to the ambulance driver, but...
He must live in a different place from me.
Ours run lights while transporting, even "non-emergency".
No lights = no patient.
The meat wagon doesn't haul deaders.

There are many possibilities:
a) someone forgot to turn on the sirens and lights
b) more than one ambulance was sent; it is an extra, leaving with no patient
c) everyone died
d) although the accident looks bad to you, the person in the ambulance was not severely enough injured to required lights and sirens
e) the lights and sirens are not working properly
f) everyone who was in the cars left before the ambulance arrived or refused to ride in the ambulance (for whatever reason)
g) no one was in the accident; the cars were remote controlled
