Family Reuniting

I relinquished my rights to my son to my ex wife and her new husband 25 years ago. His mom told him he was adopted and that I was his real dad when he was 12. We have had a great relationship and he is 29 now. What we would like to do is have him be my son with my last name. I need to know what we can do.
