Anyone have moroccan passport renewal form????

(my husbands question )hello. i have recently recieved my 10 yr visa however while my applications were being processed my passport expired, so i must now renew it by sending it to new york,to the moroccan embassy. i have a moroccan passport . on the website the application forms will not open. i have tried(dont send link wont work) does anyone have it saved in their computer in microsoft word format and can email it to me? i just need to fill it out so i can apply for my renewal thanks .it will not open on my computer in the xl format

i have tried to open the passport link also but could not,
i went to another place, i imagine all passports forms are alike?

click on form 1 , under the visa;
i have looked at most consulates in the world, have not been able to get the form nor download it.
