I would. Usually they require it so stay on the safe side and do it.

In general, you should notarize any document that says you should, either on the form itself or in the instructions.

It can be useful to notarize any supporting document you can, as well. Even though, frankly, we don't believe notarized documents any more than we do non-notarized ones, we do appreciate the effort you went to to get them done.

Look you are about to go through a process that if the paperwork on the application is not 100% correct the application will be denied.
