From where i get ds-156 form?

how ds-156 form is fill up?. & from where i receive it? I can get it after pay visa fee or before visa fee?

they're available on-line. check out the website of the US embassy or consulate in your country for more precise information on forms and fees where you live. take a look at usembassy.state.gov and look up the embassy website in your country and then click on the consular or visa section.

As previously mentioned, you need to go to the US Embassy or Consulate site for your country. But now the form is electronic in almost all cases and must be filled in on-line in English only. This will then be automatically submitted to the Consulate and you will get an interview date. You will bring in all of the other documents when you go for your interview and pay the visa fee before the interview. For a tourist visa the cost is $131 and if you don't get the visa, you lose your money.
