HELP! I'm in huge trouble!?

I need to take a medical , and in my student application form i did not write down my mailing address , but my e-mail , fax and phone numbers were.
How will the Canadian Embassy send me the "medical form" a paper i will need to take with me to a hospital if i did not give them my mailing address? Can someone please help me? :(

I will be worried if the embassy, even if they have your email, fax, and phone numbers, will not even try to contact you to inform you of your oversight. Better yet, why not call them and give them the information?**

Why don't you contact the Embassy and let them know your information has changed and that you are applying for a visa.
