coz my aunt (mom's sis) is sponsoring me to go to canada, the problem is, in her records, my mom's name is juliana, in my b.c., it's julie. can an affidavit be enough to support the correction?

You cannot change your birth certificate. You can submit an affidavit FROM YOUR MOTHER correcting minor discrepancies in her biographic data if she includes concrete documentary evidence of the correct information.

I think you need to concentrate on finding documents to support that your mother used both names, and that the documents refer to the same person. You have to establish that your mother is your aunt's sister, and that you are your mother's child. Is your mother still living? I don't know, but I think a notarized Affidavit from your mother that states your relationship might be useful. Even a letter from your aunt might be useful. It's probably something you can do yourself (documents supporting that your mom is really your mom... ex. has she ever co-signed a loan for you?), but you might consider hiring an immigration lawyer to untangle things for you.

Even if you could change your mother's name on your birth certificate, I don't think it would help you. The Canadian government might detect the change and you'd be out of luck.

If you want to change the entry in you Birth Records, so that your mother's maiden name will now appear to be "Juliana" instead of Julie, you will need to file a Petition in court for Correction of Birth Records.

You may have heard that the Civil Registry now allows non-judicial change of name, meaning that the Civil Registry will make changes in your Birth Records even without a court order. All that they will ask you to submit is an Affidavit.

However, this cannot apply to your case, for two reasons:

(1) The name you seek to change is not yours, but your mother's; and

(2) Non-judicial change of name is allowed only for FIRST NAMES, and not for any other entry in your Birth Records.

Stated otherwise, if you want to change this particular entry in your Birth Records, you have to go to court, because the entry you want to change is not your first name.

HOWEVER, for purposes only of procuring a Passport, the Department of Foreign Affairs (DFA) will actually accept a mere Affidavit of Two Disinterested Persons which will attest that the Juliana appearing in your Birth Records is the same person as the Julie whom you claim to be your mother. No need to file a petition in court if this is the only thing you need at the moment.

However, if you need to straighten your records out (and this is recommended), you will have to file a Petition sooner or later.

I don't think you can change it. Your mom would have to do that and she would most likely have to write out a statement as to why all this information was wrong in the first place.

You might be able to call the hospital and see if you can talk to the Medical Records section and see if they can pull out a copy of their records as to what she name she signed in under (this may need a subpoeana though).

I'm guessing your mom uses Julie as a nickname, the place of birth is the large town/city instead of the suburb and date of birth may be an error in that the writing was hard to read. If that is the case then the DOB can easily be changed providing you have the hospital's blessing and they provide details backing up what you have said, the mother's name would be more difficult because that was what she was using at the time of your birth (and most likely the name she checked into the hospital on) and the POB can be linked by seeing it's part of the place (POB possible.. but... can't be certain).

you can not. it is your mother who can change her first name, place and date of birth (assuming wrong entries were made). once your mother have made the necessary corrections, then you can make changes on your birth certificate too. you can do this by going to your local civil registrar and request for changes to be made on your birth certificate. the process should not take longer than a month.

let me ask you, if you were on your mother's shoes, and your daughter is deleting you as part of her life, how would you feel? Sorry if your question is answered with question, I just can't imagine how a mother felt after giving you the life, she is dumped like a rug. I pity her, really. Sooner or later, u'll become a mother too. Don't do unto your mother what your daughter don't want do unto you!

besides, do you really want to go through all that trouble? if you do, these are what you have to prepare:

1. your parent's birth certificates;
2. if you're adopted, your adoption papers to show who your real parents are;
3. affidavits for everything;
4. affidavits of third disinterested party who have knowledge of these;
5. submit these to NSO for jusgement

Why would you want to do that?

You would need to do the whole thing legally, so I recommend you contact your local registrars office (normally the town hall) and see what they say.
