You will need either proof of booked accommodation and flight tickets in & out or sponsorship/invitation by an EU citizen/organization. Depending on the embassy you apply at, you may also need proof of employment and further documentation to show that you intend to return to your home country. Inquire at the embassy in question for the exact details.

You have to submit the Schengen visa form to your local embassy. You can download the form here: http://www.euro-dollar-currency.com/schengen_visa_forms.htm . In addition to the Schengen visa form, you will also need photos, invitation, documentation of sufficient funds, schengen visa fee, travel insurance.
