i need to post an affidavit form to my mother in Pakistan.How n where will i get it from london?

If it was issued in the uk then you should be able to get a copy from your local county court office or the one nearest to where it was origonally issued

Contact the Pakistan High commission in london or the pakistan embassy to get the affidavit, duly stamped.

An affidavit is simply a witnessed testimonial account. Any Notary Public can supply you with an authorized affidavit, from there you need only overnight it to her.
