Yes, a child born today to even one US citizen is considered to be a US citizen at birth - Jus sanguinis. In addition, anyone born in the United States is considered a US citizen at birth - Jus soli.

Yes, but your parents should have registered your birth at the nearest embassy to be sure there is a record of it. Since your birth certificate says you were born in that foreign country, the registration provides documentation that you are a U.S. citizen.

YES and NO...

If your US citizen parent(s) reports birth at the nearest embassy, then you are US citizen BUT if your US citizen parent(s) fails to report birth at the embassy, you may obtain US citizenship before your reach 18 years but if your parent(s) does not help, it's very difficult.

i think it only takes one. but there is a paper that needs filled out reporting the event. you should be able to find the answer on google .
