An American passport or a Ghanian one? I don't know of any countries that require their own citizens to get a physical to get a passport. I take it your special someone has asked for help with the various fees. sometimes they claim the physical is for the US tourist visa, which also has no medical check up requirement. Be careful, this sounds a lot like you're being scammed.
