The person taking the affidavit only needs to know that the person signing is really the person whose name is written on the document. Validity of the document is the responsibility of the courts.

Sometimes. An affidavit is only as good as the affiant. If I got an affidavit signed by my brother I would believe it. If I got an affidavit from Panama Jack McCain I would need a lot more proof (the guy doesn't even know which country he was born in)!

It depends on why the affidavit is sworn and what facts it is attempting to verify.An affidavit is a sworn statement of the facts asserted by the person swearing it usually on the basis of their "knowledge and belief" or some other such phrase.If the affidavit refers to a document it would be normal to exhibit a copy of the document to the affidavit.
