Yes you do, preferably the full one not the short one. And it must not be a photo copy and they will return it. Use the passport check and send service at the post office, it costs but they make sure it is filled out correctly and that the photo is OK then send it under secure delivery. |

Send the ORIGINAL for both yourself and the baby - plus you may be required to attend and interview at your local passport office as this is the babys' first passport.
allow 6 to 10 weeks for this to be processed!! this is due to security increases caused by recent terrorist actions. |

YES... just READ THE APPLICATION.. it clearly says you need a CERTIFIED COPY OF YOUR BIRTH CERTIFICATE.. for you and the 6 year old child and you will also need a LETTER from the MISSING PARENT stating she knows you are leaving the country with the child and GIVING HER PERMISSION FOR YOU TO LEAVE THE COUNTRY WITH THE CHILD...
If you don't send in ALL the above... you WON'T GET A PASSPORT. |

If it's their first ever passport, then yes. If it's the replacement passport for years 6-10 when you have to submit a new photo, then, no. |
