You must use this form if:

You have never had a U.S. passport or
You are under age 16 or
Your most recent passport was issued more than 15 years ago or
Your most recent passport was lost or stolen. (If you are applying because your most recent, valid passport was lost or stolen, you will also need to submit Form DS-64 listed on the left.)

Because your passport was issued more than 15 years ago, you must apply in person, using the DS-11 form.

You can't renew an expired passport. You have to start all over and get a new one.
