Passport name change?

I want to change my name order in the passport. So, i need to provide my original passport along with the affidavit signed by the notary that the name has been changed. So, will i get a new passport with a mention that the name has been changed or will it be a completely new passport with only the new name?

It will be a completely new passport with only the new name.
The new passport will also mention the file no of your previous passport issued.But it does not reflect your previous name.
