If you are an American, you can either go through the steps at certain Post Offices here in the States, or you can do it in your new country at the US Embassy or Consulate that serves your area. If you do it in your new country you will need to go to the Embassy or Consulate website and follow the instructions. Basically you'll download the application, fill it out, but don't sign it; this must be done in front of one of the Consulate officers. Bring your photos and some money. It must be done in person. They'll either send it to you by courier if you choose and pay for this option, or they'll e-mail you when it's ready and you'll pick it up yourself. It takes about two weeks; faster than in the US.
They will not take your old passport from you.
Something else that you need to check into though is many countries will not accept a visa that is in an expired passport. You'll need to find out what the rules are in your new country.

if you're talking about a US passport, go to the post office and ask for the passport renewal form. it will explain what you need to do. or perhaps you can apply online - see US Dept. of State

Contact the U.S. embassy or consulate in your adopted country. You will likely be renewing through them. You might have to mail your passport to them when it is time to renew or visit the office in person.
