This is what you'll need for your Dual Citizenship:

R.A. 9225 Form No. 1
Notarized Oath of Allegiance
Two 2X2 passport pictures
Proof of payment of applications fees Php2,500.00
Proof as natural born Philippine Citizen
Photo copy of foreign passport
Photocopy of certificate of naturalization
