You will need to submit Form N-565 to the following USCIS office Service Center:
DHS/USCIS
Texas Service Center
PO Box 851182
Mesquite Texas 758185 -1182
There is a $220.00 fee. See the following site for more information:

TRY doing this first as you already have proven you are an American when you first applied for the passport.(save $220.00) It would be great if you had records of your passport details.
To Report a Lost or Stolen Passport:
Complete Statement Regarding Lost or Stolen Passport, Form DS-64
Fill in as much of the passport information as you can.
Answer all the other questions in detail.
Sign and submit above Form DS-64, Statement Regarding a Lost or Stolen Passport, to:

Go to the embassy they should help you with everything. As long as you have proof you are an American citizen and entered the country legally you should not have a problem. Take you plane ticket with you if you have to if you still have it. |
