- completed application
- two passport photos
- certified copy of birth certificate
- marriage, divorce, death certificate where applicable
- old passport
- certified copy of de-registration from your last German place of residence, if one is entered in old passport
- certified copy of US green card, visa or passport.
- copy of US driver's license or copy of utility bill as proof of address
- copy of German degrees or titles if you want them entered into your passport (Dr. Meier) where applicabke
- where applicable: certificate of German citizenship (if German citizenship has been obtained by the person), or certificate of permission to remain a German citizen if US citizenship as been acquired by German citizen (dual citizenship)

He should contact the nearest German consulate/ Embassy and follow their instructions.
They will issue him with a temporary passport. However if he is in violation of the immigration laws of your country he might have problems when crossing the border- so getting Consular advice first would be the reasonable thing to do. |
