Green Card "Biographic"?

Iam an Iraqi Refugee, When I came to USA They gave A I94, but my name was wrong they put my grandfather name in place in my surename.
after about 9 months I coreecte it and they gave me a new I94.
My question is whene i applay for a green card do I need to but my old (wrong name) in the Biographic form or not.
NOTE : Iam not using the old one any more and all my Document in the new name

Include the correct one.If USCIS will ask you for the original or the one with error then that's when you will furnish them the copy of it.

I think you can try to correct it...i would tell them my correct name to correct as long as you have papers to prove your real name, they will fix the problem. But becareful.
