It depends on the number of days the Immigration Officer gave you upon arrival in the US. You may have a 3 or 5 year multiple entry visa but the length of time you can stay in each visit will be determined by the Immigration Officer.

Depends on the country and what type of visa you have. Most work visas are valid for 2-3 years, then have to be renewed after that.
