How to find out where my mom issued my Immigrant visa?

How to find out where my mom issued my Immigrant visa?

I need this info to fill out my I-90 form but my mom doesnt remember or have any papers that say where the consulate or USCIS office was that we issued for my visa....how would I find out what office she issued it from?
