How to get passport in tatkaal scheme?

I have these related documents - Driver's License, PAN Card, Post Office Savings Account and HDFC Account. I don't have Voter's ID Card. I have to travel to US next month. To do so, I need to get Passport in one week. PLease help me providing appropriate information.
