Marriage between an American and a Canadian, marriage visa - time frame, possibilty of being denied?

My girlfriend and I have been dating for 2 years now, shes American and I am Canadian. We cannot live together due to the fact that neither of us have the others home country citizenship, but when we get married we are planning to petition to get a marriage visa.
What is the process of this? How long does it take generally? What is necessary to be approved? Will it matter if we get married in a state she does not live? On what grounds would it be approved or denied?
What we have found so far is that to prove it we may need to have a bank account together or share property, but thats not possible if we don't have each others citizenship.
Any information would be appreciated, thanks in advance!

The US immigrant visa petition is form I-130. The fiancee petion form is form I-129F. These take a few months to process. It does not matter where you marry but the fiance petition is used to marry in the US. There is no possibilty of denial if it is a bona fide marriage.
