What are the reasons USCIS denies petitions?

I am filing the forms I-131, I-765, I-864, I-485, G-325A, and I-130 to adjust the status of my husband who is legally in the country with a I-94, do i need to send his medical exam too or do I wait for USCIS to request it? and if he has no criminal record, he's clean are there still any chances they will deny the petition? what would be the reasons?

Find a civil surgeon in your area <-- google this phrase and you'll find one.
Bring form I-693, all vaccination records you can find, his passport, and some money, $200-ish if only a tetanus shot is needed. But prices vary. Ask. More necessary shots will make it more expensive.
The civil surgeon will hand you a sealed envelope with the completed form I-693, which you'll have to submit with your package. You'll get a copy for your records.
As far as your other questions go... no idea. Fake marriages for sure.
Yes, if you don't send in the medical they will ask for it, but why would you want to get your process delayed, possibly by several months? |

If you don't send the medical they'll ask for it. Don't worry about it. If it's a legit marriage, and he has no legal issues, the odds of getting denied are very low. |

You should have filed the medical exam with applications. USCIS may send you an RFE for further evidence. Consult an AILA immigration attorney for advice. When people do things on their own, they usually mess things up, much like you did here in this area of law, which is deceptively complicated. |

It is better send Medical with all your applications for the faster and smoother process. Once you get RFE it stalls the process.
If all the required forms, evidence, documents, fees are in order, you have nothing to worry.

Well, as long as your record is clear and you submit all the documents you need, you should be fine. Yes, you do need to submit your medical exam. If you have not sent it yet, you are more likely to receive request for evidence (RFE)
If you are not in rush, you can simply wait and send the medical document later. If you are in hurry, call up the customer service and talk to a rep or supervisor and ask what you can do.
