Your doctor will give you a form to fill out. Your doctor completes a section and mails it off. You will recive your disabled parking tags in the mail.

You need to have a medical to see if you can not walk far .and you must have evidence from your GP .Who will sign a form saying you are unable to walk very far ?You just can't get one that easy.Good luck if you do?

In the US, you can obtain a disabled parking permit through your state motor vehicle office. You will probably need either a letter from your physician or a form that he/she needs to fill out attesting to the fact that you are disabled.

you need to get a prescrition from your doctor and take it to the dmv thats all it takes

you need a slip from doctor stating your disabled, then take to Secretary of state,they'll give ou more information if needed. you might want to call first just to make sure
