get a form from the post office - just need to confirm your details, send your passport style photo and cough up the money. Don't forget to tick the box which promises that you'll return the old one when and if you find it down the back of the sofa. |

If you're in the UK, you'll need to get a form from the post office to tell the DVLA you've lost it and you'll need to pay for a new one, which I've got a feeling is the best part of £40. But I may be wrong with the price. |
