He can not drive my car since it is a five speed. My ex-mother in law keeps calling to tell me that he has to be put under my auto insurance, but I always thought insurance followed the car and not driver. Any advice or thoughts will be appreciated.

How many more times do you plan to ask this same question? Yes, if he lives with you and has a license, you should either add him or list him as excluded on your policy.
Just out of curiosity, have you read the community guidelines? Especially where it says:
