Got it how? That is a document you don't leave laying around. How do you know they put it in their name? You're obviously leaving a whole bunch out here. But, if what you are saying is true, call the police and start a report. Do they also have the car? How did they get it in their name without you signing off? Or is there forgery here too?

Now, on the other hand if you sold or gave away your car and signed off on it, and are having second thoughts, it will get you in a lot of trouble if you make a false report.

you will have to report them to the dvla, if they stole it possibly the police.

I would contact your local police, and hope you can prove that you didn't sell or give the car to the bad guys.

The document is not proof of ownership. It records the details of the Keeper, who is the person responsible for the vehicle on a day to day basis. It's also the person who gets the parking tickets &c., so there is no real advantage in having the car re-registered unless the intention is to sell it on. As long as you have the car and the original paperwork for buying it you can show the car is yours. To undo the trickery you need to make a police report and get an incident number, then contact DVLA with the details and copies of the proof of ownership. Don't expect it to be resolved fast. You might need a solicitor on the case if the alleged owner is awkward.

Elvis is in the building. You've got yourself one tough little problem. I would go to the police and report it right away. If they've put it in their name they might make a stolen vehicle report. If you have any paperwork (maintenance, smog, ect.) that shows you own the vehicle, I would start getting it together right now. Your going to need as much proof as possible. I'm hoping that you don't carry your title around or leave it in your vehicle. The only thing you should be carrying is your registration.

Sorry, I'm not going to be much help but I just posted a question how can I tilte a car that I bought that the previous owner did not title or register. Do you have any ideal how the person went about puting the title in their name w/o you ?
