You need to establish liability in your letter (provide them with a copy of the police report) and make sure you outline your medical treatment, itemize any medical expenses (full expenses, not after your insurance paid), itemize any lost income (have your employer write a letter and attach it to yours) and advise if you are left with lingering pain. Also, attach your medical records and bills to the letter. In lieu of attaching them, you can sign an authorization for the insurance company to obtain them, but make sure the authorization only allows them get those that pertain to the accident. Read it carefully if you are going that route.

I believe this type of letter is considered a demand letter. Basically stating everything that has changed in your life as a result of the accident. Basically this letter is going to determine how much pain and suffering you are able to get. I suggest you don't leave anything out. If you have not been able to be as social as before, mark that down. Weight loss or gain, depression, pain, medical bills, loss wages, mileage of going to all the doctor's appointments....Be as detailed as possible, but be realistic as well.

My accident was back in December and I realized about 2 months ago that I need to document what I go through for when I need this letter. Being in pain and on medications makes it easy to forget. Good luck!

Why are you sending this to the other persons' insurance? Normally this would be a form that the police would give you.

I would list all damage to your vehicle, including repair costs (if you have already repaired the vehicle). I would then list any income you have lost due to the loss of use of your vehicle. I would then list all personal injuries, and income lost from injuries. Be careful not to under or overstate all of these, as the insurance companies have people who do "spy" on people claiming injuries and damages. They will watch to see if you are injured, or if you are, how bad your injuries really are, how bad the car was damaged, etc.

Just tell them. Write what you feel, emotionally and physically. Back it up with DR's staements, if you can.

just be truthful in your statements and provide only facts. Avoid providing your opinion or 'what could have happened'. State what happened and what the result was.

ask your lawyer or insurance company, they are use to dealing with this type of stuff and can get you started and finished.

whoa woah woah ... before you do anything, were you, injured in any way??? Was it the other person's fault? If so, don't give his insurance company anything. Instead contact a lawyer.
