Police background check question?

I'm filling out a packet for my background investigator and on it there is a section about medical history. What exactly can they find about this? Like it wants to know every doctor I've ever been to. I had tendinitis in my shoulder on 2 occasions not too long ago and I went to a physical therapist for it. It's fine now but would they be able to find this out without me listing it?
Additional Details
thanks for the advice everyone. No the tendinitis wasn't chronic, it was from working out. I got it cleared up at PT tho and I made the adjustments to keep it away for good. There's another thing now that I think of it too, when I was younger I was "diagnosed" with light ADD. I've long since outgrown it (provided I ever even really had it) But with this, I have no way of finding out who the Doctor was.. not even my mom can remember who/where it was. What should I do with that?

Most like, at the end of your packet there was one page. And if its in your particular packet then its a catch all kind of page. It will most likely say that you give permission to re lase any and all private info including but not Limited to medical records. Look for this page because it will tell you everything. And remember, if they do catch you lying you can kiss your chances goodbye. Good Luck.

No,you have doctor patient confidentiality.Only way they could find any medical history is if you tell them, they get a warrant or if you sign some thing giving them the right to ask your doctor(s).

No, not without your permission. Medical records are private. However, if you have a medical condition that would interfere with your ability to do the job or which would prohibit you from doing the job and you conceal it, you may be subject to immediate dismissal if it's found out.

You should disclose your tendonitis IF it is a chronic condition. Twice is not chronic, however, tendonitis can become chronic, and if the job is going to require heavy lifting or repetitive movement of that shoulder, it's likely to become a problem for you.

Disclose everything! besides, at the end of the packet you received there should be a section where you sign a document and have it notarized so they can review your records or obtain any information they see fit.

If you are the only source they use to verify info. they would never know better. If you authorized them access to med records that's a no brainer, you have to list it.

Here's the thing though, ...with it being a POLICE background check I'm guessing there is a signature to confirm everything you state is true.
