I'm 22, so I'm over 18, was born in kenya, not the USA, so I do not have a birth certificate in any vital records office in the USA. I have been a permanent resident since 2004. I lost my original SSN card but am seeking to fill out the SS-5 form to get a replacement, because I'll need one.
What do I put for the sections 8A and 9A where they ask for my father's and mother's names if you dont know them?

I was abandonned by my parents when I was in kenya as a little child. Some people took care of me when they found me and took me to an orphanage. Around when I became 11, I was brought here to the USA by a man who cared about me. I wasnt adopted but this man became my legal guardian as he took care of me till I turned 18. After that point he had to return to kenya, and I got a job and supported my own self. I got my SSN in 2001 when I was 15. For the life of me, I have no idea what he put down on that form when he applied for my SSN. So that's the problem.

Why don't you know their names? Who filled out the first form to receive your SSN? Ask that person if they remember what names they used. Regardless, you will need original documents. Here is more about that: http://ssa.gov/ss5doc/?ID=rep&Selfchild=self&Status=res&Submit=Submit
I can't imagine you don't have the names of your parents on any original documents. If you were adopted or something, they would go by your host/adoptive parents. |
