Yes, but it's very rare.
Usually only happens if the adoption can be proved to be somehow fraudulent to begin with - for example the birth parents consent was not properly obtained.

Sometimes. The only ones I've seen are cases in which the parental rights of the birth parent were terminated, but the person was not properly notified. |

I didn't think it was that rare actually.
There was a case in my hometown several years ago where a couple legally adopted a boy at birth. 6 years later, the woman who gave birth to him fought to get him back. She said that when she lost custody she was a drug addict and because she was on drugs she could not have made an informed consensual decision to give him up. She has since been in and out of rehab several times, lost track of the man who impregnated her, and lives with another man. Her life is on the fringe of credibility.
The couple have spent hundreds of thousands of dollars disputing this case, and at 8 years old the courts gave custody to the woman who gave birth to him. Isn't that insane? He had to leave a loving family in a wonderful home who were the only parents he ever knew... and go live with strangers because they shared the same DNA. The courts are run by idiots with no common sense. Obviously a drug addict living on welfare isn't the best parenting material. But the judges gave the boy to her anyway. The couple lost every appeal and they took it 1 step below the Supreme Court before they gave up.
