Daughter's name change.......?

My daughter is 13 and wants to change her surname from that of my Ex-husbands to incorporate my maiden name which i am going back to - so it would be {her name} [my maiden name ]-present surname.
My ex- husband is fine about this as he knows how much it means to me,but how would i go about doing this in the eyes of the law?
Any advice would be greatly appreciated.

Hi,
A deed poll document will enable you to change your daughter's name to incorporate you maiden name and her father's surname. As long as you and your daughter's father are willing to agree to the change and sign the Change of Name Deed there should be no problems with getting all her records updated.
If you are planning to change your daughter's name it is very straightforward and not as complicated as you may think. You can apply for a Deed Poll to get all her records updated to the name for just £38.00. This can be completed online.
A Deed Poll is accepted by all UK Government Departments and organisations, including the passport office.

Once this is complete you can use the Deed Poll to update and inform everyone of your name change. If you need more than one copy of the document, you can also order extra copies for £2 per copy. Once this is complete you can send the deed poll to the passport office to get your daughter's passport changed to reflect her new name.

You need to get a solicitor and sign a change of name deed. It will cost though. I don't know how much because I did it in 1991. |

You need to go to the county court house in the county where you reside and get a form for the change, you and your ex husband fill it out, sign it, pay the fee and they will give you a date to go to court, have your ex go with you if possible and the judge will grant it, you do not need an attorney for name change if you are in the US.~ |
