Does UK institutions for example; police, hospitals and work need knowledge of a person's marital status in the UK.. or can the marital status remain private?

They need to know who your next of kin is, so that they have someone to contact in an emergency (e.g. if you have a freak allergic reaction to a dental anaesthetic, collapse unconscious & are taken to intensive care). I believe (but am not sure) that in law a married person's spouse is held to be their next of kin. This may not apply if the couple are separated.

If for some reason you would not want your spouse contacted by the dentist or doctor in an emergency, you can tell them so and they should go along with that, and should keep your request confidential. (NB this may not apply if a person is being treated under the Mental Health Act, in which case the doctor must give information to the 'nearest relative').

Marital status is private in the UK, the only issue is if you are an immigrant and may not have the right to remain which may depend on your family arrangements, otherwise if you are a UK national it makes no difference.
