How do I report a pharmacist that is under the influence of drugs?

I was a patient at a rehab hospital and while I went outside to meet my husband we noticed I seen one of the pharmacy ladies in her car snorting and smoking whatever it is that needs to be smoked out of a pipe, during her lunch break.

Each to their own I normally say.

But In my case I had my meds mixed up with another patient which I was allergic too so good for me I noticed the mistake. I am sure it was the fault of the pharmacist I am speaking of since she was the only one in the lab when I passed by.

I'm concerned she'll cause another mistake that another patient won't catch in time.

I have asked to speak to the head of pharmacy department but I'm getting the run around.

I have the pharmacists' name on a paper she once signed off for me before.

Is there a place I can contact... email/call that deals with pharmacy licenses?
or any kind of help???

Every state has a State Board of Pharmacy. They regulate both pharmacies and pharmacists.

That's who I would contact, almost all have websites.

Rehab Hospital? -- Write it all down, dates and time and the name of the pharmacist and your name & address and mail to the Hospital adminstrator (call up the rehab and ask for name of Hospital administrator) Say in that letter that a copy is being sent to your PHARMACY BOARD for review. Look in your Phone Book - for STATE Board of Medical Practices - its' there somewhere. The next mistake this pharmist makes may be fatal - don't play with other peoples lives. REPORT HER!
