How does a case get to move to an appellate court?

If the defendant is found guilty in the appellate court, is it all over? Or is the next step up the supreme court?

It is not that simple. At the state level you have three main categories. superior or trail court; state appeals court and the state supreme court. Theoretically, you go through the state process (state supreme court is discretionary in most cases, i.e., they may refuse to hear the appeal). If there is a federal constitutional issue involve you can also go to the federal district court, or directly to the federal appeals court if you have already been heard at the state appeals court and eventually ask for a hearing by the U.S. Supreme Court. About one out of a 100 cases are heard by the U.S. Supreme court, once they make it up there.

In between, there can be various motions to reopen or retry a case; because of some significant errors that may have occurred, usually involving admissibility of evidence or some new and material evidence is discovered. Things can get very complicated.

Every case has one shot at an appeal by law. After that the court may choose on its own whether to hear a case.

Appellate courts do not re-try a case. They only review the legal questions and mistakes of law raised by the previous hearing. It is not their job to determine facts of a case, but to ensure the law was applied properly.

Depends. The defendant can appeal the appellate decision, but the supreme court does not have to take the case.

The Supreme Court does not choose on its own to review the lower court decision. There must be an appeal. However, once the appeal is made, they can choose whether they hear the case or not.
