If you see faulty equipment, then you are legally obliged to report it. That is criminal negligence and jail time could follow. You always inform before an accident if you can. Then you are never in a position of I should have done this...
People are entitled to their own opinion. If it's a popularity issue, then I won't interfere. If it's character destroying gossip, then I will defend the cousin. Facts should be correct.
First rule of any rescue operation is NEVER become a victim. Placing yourself in that position unnecessarily risks your life as well as any who try to rescue you and the person you tried to save. Assess risks before you lose your life in vain. That's what trained personnel are for. If I were properly trained and equipped, then that is different. If it is reasonable that I can succeed, then that is different. When in reasonable doubt, don't risk it.
The same rule applies. Sometimes we do things we know we should not. For example, I would place myself in harm without thought to protect my wife. It would not even be a thing to think about though I should....at least I probably should. I would go to war and commit acts to defend my country. I would commit terrible acts to defend my family. It's not necessarily what you do. It's why you do it. Otherwise every soldier would be jailed.

1. NO -- Their mistake, not mine; and they're not about to compensate me for my time and effort.
2. NO -- Such a sudden change in performance would raise too many questions; and if I'm failing, it's because I'm not putting in the proper effort.
