What title do you use for a contract?

I'm writing a contract for a design company. It is the designed sending the contract to a client to sing, detailing what they can do, what they'll have rights to and deadlines for payment etc.

I've no idea what it's meant to be formatted like so have hazarded a guess. But the one thing i'm really stuck on is what title to put? My boyfriend put "(My Design Company) contract for Terms and Conditions" but i wasn't too certain..

I highly recommend you retain an attorney to draw the contact for you.

Your clients' attorneys will easily find loopholes in a contact you've created yourself.

That is o.k. There are no is no sacrosanct and necessary nomenclature for the document if it sets forth terms and conditions to which the parties sign. "Contract" in the title is sufficient clarity to indicate that the document is intended by the parties to be a contract, binding and enforceable. The terms and conditions must be set forth with sufficient clarity to bind the parties.
