Where do I obtain proper petition/application form for requesting expungement in texas?

if anyone has done this and can tell me the procedure it would be appreciated. I have been told you can take the action yourself without hiring a lawyer.

The best first step is to try your county clerk's office. They often have forms and petitions on file and can give you a copy, or at least tell you where to go to find one.
If you don't get what you want there, call up the Department of Public Safety. If there's an application out there, they probably have it.
Last but not least, there may not be a specific petition or application out there. Reading the statute, it looks like you just have to file some type of motion or petition with the court. There are tons of motions used every day that you can't get a proper form for. You may just have to draft your own.
