Help im in limbo! My manager wont fill in a form?

My manager made it hard for me to stay in my job so i handed my notice in but only gave her a week not 2 like she insisted. Now shes gone on holiday without filling in a form about my last 5 weeks income. I cant claim any money to live off till i find a new job. What can i do? Im suppose to have left by the time she get back.

shes got to fill in a run down and she wont do that as she'd have done it before she left. Care managers not allowed to fill it in either and she been arkward as i didnt give her the notice she wanted because was there 12 months but never given the written contract i was promised

Who has she left to manage while she's on holiday? Take the problem to them. If they can't make a decision they can contact her and fax or email the paperwork for her signature.
