Yes, She owes the firm the hours she has not made up yet. If she had done them earlier you wouldn't be asking this ;D

Sounds weird to me: Flexitime - is a short term practice, usually the interval between the balance (make up hours) is about weeks.
Making up working hours for last year doesn’t sound right, as there are cut-off periods for example, end of financial year (5 April) or end of the calendar year (31 December). It’s like carrying unused holidays; at one point of the year you’ll have to decide whether you take it or get paid for that. Is very difficult in the financial books take it across another financial year.

Yes, because...

1.Either she has been paid for the hours but didn't actaully do them (in which case this is theft)

2. Her contract states that she should work so many hours and she hasn't (in which case she would be in breach of contract)

Depending on her contract of employment and what shift pattern she is working to yes if she has been paid for the hours then she has to make them up.
There are lot of companies now that run flexi needs of th business shift patterns in which this is becoming the norm.
But if your girl friend has not signed up for this type of contract and is a normal 9-5 worker then it is bad management that has allowed her to take more time off than she is entitled to. If this is the case then tell your girlfriend to come to an agreement with the company of paying back so many hours per week. Or giving up some of this years holiday entitlement.

Proably - this sometimes comes up where a person has taken more than their official holiday entitlement. now and then comes up with people on flexi-time where they have been a bit too flexible and have not put in the contracted hours.

it seems weird to me, you wouldnt have to do that where i work, i think its a load of cobblers.

Why don't they just dock her at last year's rate? That seems weird to me.

And unless she's exempt from OT pay, I believe she has to be paid overtime if she works over 40 hours in a work week, whether or not the extra hours are for time missed last year. I don't think they can get around that.

Wow. I've never heard of anyone doing that.

If she is on salary, and she was paid for her regular amount of hours, then yes, she has to make up the time. She should check her contract/HR documentation.
