I had surgery March 13th and missed a couple days of work because of it. I am still having some health issues that the doctor is running more test for. I am still having a lot of pain and I have talked to my boss about cutting back on my hours until I am cleared by the doctor, but she will not change my hours. I have turned in my doctor's notes for every day that I have missed and now my boss is saying that she has not seen a single note from the doctor. My boss wants documents from the doctor on exactly what was done during the surgery, what text have been ran and the results of those test, and how long it will take to fix whatever is wrong with me. I have to give these documents to my boss by noon on Monday or I am out of a job.
Background information:
I work at an emergency clinic for animals. My boss is just looking for a reason to fire me or trying to make me quit. She has given me written warnings and put me on probation for little things like putting hot check warning notes on checks when the customer says the check will not clear the bank, and for getting to “emotional” when someone cusses me out over the phone or in person because they can not afford to have their animal seen at the clinic and we are not allowed to give medical advice over the phone. I have requested days off for family events, such as my cousin's wedding, and my boss will not let me have the days off. I had a trip planned to go out of state and had requested off for a weekend, my boss had not put me on the schedule for that weekend then called me the morning I was supposed to leave to let me know that I had to work that weekend or it would be put down as a no call/no show and I would be fired.
Is it legal for my boss to demand the documents showing what tests have been done and the results and details of the surgery that was done? Does that not violate my rights? What can I do?
I am at the point where I hate my job and it is keeping me stressed out but it is not easy to find jobs right now so I don't want to quit and being fired looks bad for future jobs.

Sounds like your boss either never heard of the FMLA ("Family Medical Leave Act") or is ignoring it. Big Mistake.
Familiarize yourself with it ASAP. You're protected under that law with your Dr's note. If she disregards his orders - shame on her, because she could be slapped with violating a Federal Law.
She's claiming she "Hasn't seen a single note from your Dr.?" Have your Dr. put in writing his request for shorter working hours; also ask him to write: "Given to (your boss's name/title here) on (date) and have her initial it in your presence. Make copies, give her the orig., keep copies.
Document any increased hostile behavior towards you, as well as what you've described above, complete with dates, her full name/title, place and what occured. Document like crazy and continue to do the best you can. Tell whoever's over her and back it up with your documentation. You're in a hostile environment and she's a Control Freak.
Tests are confidential and should only be between you and your Dr. If he breaches that confidentiality, he's in trouble. It's not her business to know the details.

Document everything. Do not stress. You will get sicker if so. Simply look for other jobs while you still remain there. Speak with your doctor and simply represent all the doctors notes. No test results are needed. Just the notes.
if you do require shortened hours, have your doctor simply state so in a letter to your boss.
You need to become valuable to yor boss, so that they will accomodate you in any way possible.
