What can I do to get my last salary that W.H.Smith still hasn't paid after 2 months of my resignation?

I was working weekends at W.H.Smith as a shop assistant. After work, on Sunday the first week of the month, I handed in the resignation. My manager promised that at the end of the month I will be paid for that week plus all the paid holidays that I had left. It has passed more than 2 months now, but I still haven't received my money. As I call them, manager is never in. I have moved cities and can't go there myself. Besides, I also need my P45 form.
Can anyone suggest me what to do? Please, I am getting desperate.
