OK, what kind of business??
Do you fall under OSHA construction guidelines
Do you fall under MSHA?

Most business that don't fall under a strict set of rules, can follow standard DOL / OSHA general duty guidelines.

Normally, NO one is REQUIRED (unless specifically directed by regulation) to be first aid trained. Only that first aid be available. (IE 911)

Many companies exceed this requirement by having a dedicated Safety Supervisor or Director, and offering First aid and CPR to their employees even thought they don't have to.

Of course, each state has regulations that must be followed as well (CAL-OSHA)

I hope I did not confuse you too much.

Dependant on the nature of business and the risks that employees maybe exposed to the correct recommendation is 1 trained first aider to every 50 employees.
The companies risk assessment should determine the requirement and if it is likely that someone could injure themselves whilst at work then cover should be provided at all times.
A first aider has to be trained by a reputable training provider and the course is for four days, they are then qualified for three years (UK) . The appointed person training course is for one day and mainly concentrates on how to take charge of an emergency situation. A good employer should assess the needs within his business from his risk assessment and train people accordingly.
In a small business it is normal for a manager to be a first aider.

Depends on the type of business. I have linked the HSE website which has a handy table.

One first aider to 50 staff.

You should have appointed persons whcih only requires 1 day training though if under 50

I think it is any company with more than 7 full time staff. An appointed first aiders course takes a day to complete and is very useful, although I hope that I never need to,
