Who are the actual guys who sit around a table and decide if and where the US will declare or end a war?

Somebody has to decide 'gee, let's go to Iraq' or 'gee, maybe we should send some troops to Rwanda' or 'how about we invade xyz' and also they decide 'ok, in 8 months how about we pool out, or lets tell them 2 months" etc. Then they write up a bill and present it to the house and then the senate then finally the president to sign. So, in the early stages, SOMEBODY has to think of it up while the rest of us are going about our day. WHO EXACTLY decides this? The army generals carry out what these guys tell them to, the army generals don't say gee let's go invade xyz and give the bill to the government so we can go do it since we feel like it. So it's not the military who DECIDES, and it's not the legislative branch (congress/senate/president) who only vote yay or nay. Who actually makes and comes up with the decision to begin with?
Additional Details
Another question I have along this line is, who decides for example where and when the President or Secretary of State will visit a country? I don't think Obama woke up one morning and thought gee why dono't I go to visit the Queen of England next week and then how about a pit stop to Turkey. And I don't think Hillary while having toast thinks hmmm why don't I go to Thailand in a few weeks and nah I don't feel like going to Europe, Maybe I'll visit President so and so in the middle east in June. I doubt either of them make those huge decisions. So who gets to decide?

For the United States, Article One, Section Eight of the Constitution says "Congress shall have power to declare War."
Despite the constitutional requirement that Congress declare war, in practice, formal Declarations of War have occurred only upon prior request by the President.
So basically, Congress can declare war but never has. Its always been at request of the President.
